REQUEST FOR ASSISTANCE INSTRUCTIONS​

 

1. ONLY A CHIEF, COMMISSIONER OR BOARD PRESIDENT  MAY APPLY FOR ASSISTANCE, ALL contact information must be included. ​

2. Failure to complete a request as outlined will result in the Terry Fund discarding the request.

 

3. REQUESTS are only accepted FROM fire and EMT/ambulance organizations.

 

4. Simple requests for gear and tools require an email or the completion of form on this page to the Terry Fund explaining need and who is requesting with full contact information.

5. YOU ARE REQUESTING FROM US.  WE DID NOT CONTACT YOU,  WE WILL NOT RESPOND TO YOUR REQUEST TO BE EMAILED, CALL YOU ON THE PHONE OR TEXT YOU.

6. You are requesting a professional grant request.  All request without proper clear and precise English, grammar, spelling and punctuality will be immediately ignored and deleted.  If you would not submit this to the Federal Grant system, do not submit this to us.
