ONLY A CHIEF, COMMISSIONER OR BOARD PRESIDENT MAY APPLY FOR ASSISTANCE

All requests for financial assistance must be submitted on a department letterhead and signed by a person of authority such as Chief, President or Commissioner explaining what you are requesting and why, and then MUST BE MAILED AND SENT BY UNITED STATES POST OFFICE MAIL States Post Office Mail
ALL contact information must be included.
Failure to complete a request as outlined will result in the Terry Fund discarding the request.

REQUESTS are only accepted FROM fire and EMT/ambulance organizations.

An additional terms of donation document must also be completed and sent back to the Terry Fund prior to receiving donation
